For adult family members, I confirm they have agreed to this; for children, I give consent as parent or guardian.
I understand that this information, including messages, test reports and payment details, will be seen only by the chosen doctor and by the consortium administrator for support and complaints, and will not be sold or shared for advertising.
I agree to receive verification codes and consultation updates on my registered WhatsApp number and email.
I confirm that the information I give will be true and complete, including present medicines. I understand that the consultation fee is paid directly to the doctor, that the consultation period starts when the doctor verifies my payment, and that I must renew after it ends to continue.
I understand that online consultation does not replace emergency medical care. In an emergency, or if symptoms suddenly get worse, I will go to the nearest hospital.
I may ask the consortium to correct my information or to withdraw my consent and close my account at any time; medical records may be kept as required by law.
